Please be sure to print a copy of this "THANK YOU FOR YOUR ORDER" page. You will need to submit a copy of this page with one of your rebate forms. You will also need other documentation, as shown on the rebate forms.

Other guidelines

  • Your paperwork may indicate "OrderVoIP", "OrderPacket8", or "OrderDSL". These are all valid.
  • You will find complete instructions on the rebate forms. Please be sure to read them and follow them to ensure validity of your submission.
  • Be sure to submit your rebate forms within the required timeframe.
  • Be sure to read the instructions and Terms & Conditions on the rebate forms.
  • You may see the designation "OrderDSL" in your confirmation form or email. This is a reference only; you do NOT need to switch your broadband provider to DSL if you currently use a cable modem.
